RESUMO

Aiming at the shortcomings of traditional medical device tracking and supervision mode, such as delayed feedback of medical device positioning information, poor visual tracking effect of medical device, and missing early warning of medical device working state supervision, a medical device visual tracking and supervision system based on wireless Local Area Network (LAN) real-time positioning system is developed. Introduction of wireless LAN real-time positioning system, using standard coding algorithm generating device object ID tags, with the only attribute based on RFID read-write terminal complete physical mapping of medical equipment and material ID label, we realize a real-time positioning and traceability of medical equipment, with the help of active warning algorithm based on depth of learning medical equipment working state supervision for early warning, with the help of indoor map generation and trajectory playback medical equipment to realize visualization back regulation. Choose equipment of hospital of Linyi Central Hospital for performance evaluation of the carrier, the evaluation model of engineering practice and clinical statistical analysis, the results show that the system has a complete real-time positioning, visible trace, abnormal warning function. In medical equipment real-time positioning information visual tracing feedback, abnormal state warning etc, medical equipment has obvious advantages.

Introducción Los avances tecnológicos alcanzados en relación con las comunicaciones y la información se vieron potenciados luego de la incorporación de la Web 2.00 y el networking impactando las redes y comunidades virtuales. Los dispositivos con tecnología inalámbrica han impactado la eficacia en las comunicaciones, que facilitan el desarrollo de tareas diversas, ventajosas por su repercusión en la economía, imagen y competitividad. El acceso a dispositivos inteligentes se ha extendido en la población ecuatoriana, donde se trabaja por el reconocimiento a los derechos de los pueblos y comunidades indígenas al respeto de sus culturas, tradiciones, costumbres, valores, saberes y formas de vida, siendo Otavalo una de las ciudades de mayor presencia indígena del país. Objetivo Exponer cómo incorporar la tecnología en lo cotidiano con fines sociales, así como también potenciar el desarrollo y los emprendimientos locales. Metodología A partir de entrevistar al Coordinador Cantonal de las comunidades indígenas otavaleñas, interesado en divulgar e impulsar el desarrollo local, se le explica de las facilidades que brinda la herramienta de la Red Local inalámbrica Kimera, que utiliza software libre y contenido abierto. Luego de un intercambio con la comunidad de Gualsaqui se obtiene el consentimiento y se completa la información de su interés a divulgarse. Conclusiones Se conforma una red local inalámbrica que constituye una propuesta para la divulgación de la historia, sistema legal y datos de la comunidad, información que es de fácil actualización por no exigir de conocimientos técnicos específicos para su personalización(AU)


Introduction Technological progress in relation to communications and information were boosted after incorporating Web 2.00 and networking impacting networks and virtual communities. Devices with wireless technology have impacted the effectiveness of communications, which facilitate the development of diverse tasks, advantageous for their impact on the economy, image and competitiveness. Access to smart devices has spread throughout the Ecuadorian population, where work is being done to recognize the rights of indigenous peoples and communities to respect their cultures, traditions, customs, values, knowledge and ways of life, Otavalo being one of the the cities with the greatest indigenous presence in the country. Objective To show how to incorporate technology in daily life with social purposes, as well as to promote development and local enterprises. Methodology After interviewing the Cantonal Coordinator of the Otavalo indigenous communities, interested in spreading and promoting local development, he is explained about the facilities offered by the Kimera Wireless Local Network tool, which uses free software and open content. After an exchange with the community of Gualsaqui, consent is obtained and the information of interest to be disseminated is complemented. Conclusions A wireless local network is installed that constitutes a proposal for the disclosure of the history, legal system and community data, information that is easy to update because it does not require specific technical knowledge for its customization(AU)

RESUMO

Electromagnetic interference (EMI) can affect various medical devices. Herein, we report the case of EMI from wireless local area network (WLAN) on an electrocardiogram (ECG) monitoring system. A patient who had a prior myocardial infarction participated in the cardiac rehabilitation program in the sports medicine center of our hospital under the wireless ECG monitoring system. After WLAN was installed, wireless ECG monitoring system failed to show a proper ECG signal. ECG signal was distorted when WLAN was turned on, but it was normalized after turning off the WLAN.

Point-of-care (POC) testing is desirable because of both the ease with which it can be administered and its short turnaround time. However, because standard POC devices cannot transmit test results automatically to a laboratory information system (LIS), each result must be recorded by hand. This inconvenience not only increases the possibility of clerical errors, but also limits the proper use of test results. If POC test results are not saved in the LIS, it is hard to either monitor patients' health trends or to quality control (QC) the test results. In this paper, we describe how we have solved these problems by connecting 250 POC blood glucose test devices to the LIS via a local area network (LAN). After connecting the POC devices (we used the Accu-Chek Inform II; Roche Diagnostics, Germany) to a manufacturer-provided POC data management system (Roche's Cobas IT 1000; Roche Diagnostics), we developed our own interface program for delivering data from the Cobas IT 1000 system to the LIS. By installing a program to scan the identification barcode worn by patients on their wrists, network-connected POC devices enable users to omit extra ordering, receiving, and recording processes, and they also reduce the possibility of patient misidentification. Such a system also provides an effective way for physicians to follow both the current and accumulated test results of patients. We note that performing QC on glucometers and the sending of data via LAN to the LIS are necessary steps to monitor both patients' results and the QC of those results.

Relocation which is a well-known determinant of malaria in non-immune travelers to malarious areas; has also been found to be a risk factor for malaria among semi-immune persons who change locality within endemic regions. Further research evidence suggests that the higher transmission intensity at the travel location is an underlying factor which may indicate exposure to new variants of P falciparum for which specific immunity is lacking. This study was conducted to determine if recent change of locality increased the risk of malaria fever among semi-immune National Youth Service Corps members serving in a district in southern Nigeria.Method: Ninety six corps members who had just been posted to Ahoada-East LGA in southern Nigeria and 83 others that had been in service for six months; were followed up for malaria fever in a prospective cohort design over a 3 month period from September 2010 to December 2010. Active surveillance was used to obtain reports of fever among the cohort; followed by medical examination at designated local health facilities.Results: No significant differences in age and use of protection against malaria were found between the study groups. The incidence proportion of malaria for the recently relocated group (34) was significantly higher than that of the resident group (15.7). Recent change of locality was more associated with malaria fever (RR 2.19; 95 CI 1.243.88; AR 54.4).Conclusion: The recent change of locality within an endemic region; which serves as an indicator of exposure to new variants of P falciparum; for which specific immunity is lacking; is a significant risk factor for malaria fever. Protection against malaria is recommended for semi-immune individuals moving to new locations with significant risk of malaria transmission

To address several hospital network issues, this paper discusses the overall plan, design and renovation of hospital's Local Area Network, making full use of existing network facilities. The techniques, such as Physical Separation of Internal and External Network, HSRP, OSPF, All-Routers Networking Model, etc., create features of extensibility, manageability, high safety, stability and so on to the overall network, and provide a reliable network platform to the function of the information systems.

OBJECTIVES: The purpose of this study was to develop a prototype wireless local area network (LAN)-based tracking system and evaluate its efficacy. METHODS: A wireless LAN-based tracking system was developed with a personal digital assistant (PDA) having a simple text messaging function and a prototype stand-alone tracking device. Evaluation of the effectiveness of the tracking system was performed in two ways. Twenty-five messages were sent to nurses by direct communication and 46 messages were sent by the wireless system. Thirty cases by nurses and 30 cases by the wireless system to locate hospital equipment were performed. The time required to transfer messages and to locate equipment was measured and analyzed with a Mann-Whitney test and a paired t-test, respectively. RESULTS: The mean time required to transfer messages by direct communication and by the wireless system were 37.92 +/- 19.19 seconds and 30.65 +/- 9.80 seconds, respectively which were not statistically different (p = 0.108). The mean time required to locate equipment by the nurses and by the wireless system was 234.00 +/- 59.99 and 23.97 +/- 6.17 seconds, respectively which was statistically different (p < 0.001). CONCLUSIONS: The wireless LAN-based tracking system can save time for nurses to communicate and to check for the location of equipment in wards which allows nurses to spend more time and attention to patient care and safety.

RESUMO

This article describes how to construct a integrated wireless clinical information system based on the original information system. It is constructed on the basis of the hospital HIS through the middle ware which provides a data-interaction platform for all the existing data and the data which would be added in future for various information management systems. The software system adopts the Web Service technology in information services. The SOAP protocol is applied to the data interaction between the foreground and the background. The mode of the network structure is "Wireless Switchboard + Intelligence antenna". Relying on the wireless network, the handheld data terminal and the barcode, the network information can be extended to the patient bedsides and the mobile medical personnels.

Nosocomial infection management is one of the most important subjects in terms of risk management in a hospital. To achieve the nosocomial infection management appropriately, both public relations and information gathering are important. When it comes to publicity, it is desirable that information is offered quickly and efficiently and the personnel are able to access the information easily anytime and wherever they are. As for information gathering, it is also important to collect the information concerning the nosocomial infection promptly, and to tell it quickly to infection management persons including infection control team members. The infection control committee of our hospital has engaged in both publicity and information gathering by using the committee web site established in the LAN of the hospital since January 2004. The web site carries notifications, organization chart, list of committee members, various manuals, guidance of educational activities, infection management, etc. It is also used for questions and answers, and gathering information concerning nosocomial infection. The number of times the web site was accessed was about 200 every month, and was considerable at night and on holidays. This method of using the web site seems to be useful to promote nosocomial infection management.

Esta se soporta en el software libre e integra herramientas e-learning estandarizadas con otras de fuentes abiertas para el trabajo colaborativo, la sindicación y la gestión docente descentralizada. Se presentó la experiencia cubana de compartir en red, los procesos formativos y las fuentes de información que se desarrollan en los propios servicios asistenciales e instituciones académicas y científicas en salud. Este modelo de aprendizaje en red (MAR), partió de una concepción pedagógico-tecnológica que favorece la integración de las actividades docentes, presenciales o no, con la práctica en salud. Los diferentes contenidos, servicios y herramientas presentan una organización abierta para el aprendizaje, en el que la interacción e independencia caracterizan los procesos de educación en red


The Virtual University of health (www.uvsl.sld.cu) constitutes one of the strategic projects of the Cuban network known as Infomed. It is supported on free software and links standardized e-learning tools with other open sources for co-operative work, syndication and decentralized educational management. The Cuban experience of sharing in network the formative processes and the information sources that are developed in the medical assistance services of academic and scientific health institutions is presented. This learning in network model stems from a teaching-technological conception that favours the integration of educational activities, either face-to-face or not, and healthcare practice. The different contents, services and tools have an organization tailored for learning in which interaction and independence are characteristics of the education in network processes.

The electronic file is a reality in medical practice nowadays. We have a decade of experience with electronic files in a neonatology unit. We use a local network that consists in one server and 8 connected computers, distributed in the hospital. Filemaker Pro® is used as database administrator and access to data is protected with passwords. Data entry is made by health care professionals in charge of the patients. Patient's reports and statistical information are based on data entered to the system. This methodology allows to have update clinical data, indexing of information, to maintain track of pharmacological indications, prescribe parenteral nutrition and obtain information for research purpose. It is possible therefore, with a minimal computing expertise, to devise electronic files that can improve the quality of health care.

BACKGROUND: Point-of-care (POC) testing is expanding because of the many advantages, such as faster turnaround time, immediate decision making and patient management. However, POC testing has problems; poor maintenance and quality control of devices and management of test results. We installed data management system (DMS) for POC blood gas analyzers using local area network for the resolution of those problems. METHODS: We connected nine POC blood gas analyzers Rapidpoint 400 (Bayer Diagnostics Ltd., Newbury, UK) to POC data manager Rapidlink (Bayer Diagnostics Ltd.) by device interface using local area network and developed in-house program for connecting POC data manager to laboratory information system (LIS)/hospital information system (HIS). We surveyed user acceptability and reliability of test results. We examined patterns of problems detected and solved in operating DMS based on quality records. We calculated the change of the yearly test numbers of arterial blood gas analysis (ABGA) after installation of DMS. RESULTS: Test results of POC blood gas analyzers were transferred to LIS and could be checked in HIS. By means of user survey, we judged that the users of POC blood gas analyzers operated the devices with ease and thought test results reliable. POC data manager server implemented in central laboratory enabled remote maintenance and quality control of devices with little workload. POC ABGA test number increased by 3.7 fold during the two years. CONCLUSIONS: We developed DMS for POC blood gas analyzers, enabling test result retrieval and remote maintenance and quality control of devices. This is very informative study for other hospitals installing DMS for POC testing in Korea.

OBJECTIVE: Telemedicine uses common technologies that provide a conduit for information exchange between physicians nurses and patients. In addition to patient records, medical professionals can obtain vital signs and other reference data through telemedicine applications. also, recent advancement in RF technology and wireless communications has enabled the development of noble networks. Mobile telemedicine involves more than just communicating via mobile phone of PDA that sends and receives medical data on Portable Internet. METHODS: This paper describes the design of a mobile telemedicine PDA prototype based on Embedded Linux that can be used to monitor the medical information using a single board computer(SBC). RESULTS: The device hardware architecture consist of signal conditioning circuit, single chip micro-controller, LCD display and wireless network interface card(W-NIC). Embedded Linux was ported as its operating system and application software was developed QT/embedded 2.3.7. CONCLUSION: This system would be very useful for patients with chronic illness, especially in an emergent environment, and for containing medical cost in the aged society.

RESUMO

Este artigo destaca o papel das redes na mobilização dos recursos e das competências locais para a promoção do desenvolvimento local sustentável. Para permitir uma maior compreensão do sentido de desenvolvimento aqui tratado, aborda a Gênese do conceito, assim como sua transformação ao longo do tempo, buscando mostrar a passagem da visão clássica de desenvolvimento para uma noção mais contemporânea, onde termos como a sustentabilidade, a ênfase no local, a promoção da cidadania, da participação e dos valores funcionam como adjetivos para o desenvolvimento. Fazendo uma ponte entre a literatura sobre redes e os trabalhos recentes sobre desenvolvimento, o artigo apresenta algumas variáveis de análise que podem auxiliar no estudo e compreensão dessas novas formas organizacionais e em especial na sua gestão. Destaca, finalmente, alguns desafios e limites para estudos e experiências que buscam fomentar o desenvolvimento local sustentável por meio do fortalecimento das redes, e chama a atenção para o caráter particular de cada realidade e a impossibilidade de se tratar o tema a partir de generalizações e universalismos.
